The beginning of a new year in the ambulatory surgery center industry is rapidly approaching. Along with a new year, comes a series of changes impacting ASC workflow and reimbursement. Three industry experts outline five of the biggest coding and billing changes to anticipate for 2015. 

1. ICD-10. When signed into law in April, the Protecting Access to Medicare Act of 2014 effectively delayed ICD-10 implementation until Oct. 1, 2015. Though the switch to the new code set will take place 10 months into the year, the ramifications make it one of the biggest changes to anticipate — the shift from approximately 14,000 ICD-9-CM codes to 68,000 ICD-10-CM codes is no small step to take. "It could take [ASCs] six months to a year to get up to speed," says Michael Orseno, revenue cycle director with Regent Surgical Health

How hospitals can still leverage ICD-10 delay

A new survey shows that the ICD-10 delay hurt providers’ preparedness for the transition. But there’s still plenty of time to use the delay to your advantage. 

The preparedness survey comes from the Workgroup for Electronic Data Interchange (WEDI), which polled 324 providers, 103 payors and 87 health IT vendors. Overall, the study found that vendors and payors are more prepared for ICD-10 than providers.

In my last article on ICD10 Watch, I discussed the idea of continuing the implementation of ICD-10-ready documentation systems.

I touched briefly on reasons to continue with ICD-10 implementation — mainly the ones keeping many physicians awake at night (other than patients calling or an overnight shift).

But what about some of the more subtle reasons that come from taking a proactive rather than reactive approach to clinical documentation and (may I say it?) the revenue cycle.

Seventy-one percent of hospitals plan to enlist an ICD-10 clinical documentation improvement (CDI) services partner by the third quarter of 2015, according to a new Research and Markets report, to help them meet more stringent documentation requirements ahead of the October 1, 2015 switch to the new code set.  A number of hospitals are considering outsourcing their coding and CDI efforts as a way to ease their ICD-10 burden and help mitigate some of the anticipated challenges to revenues and productivity as time ticks down towards the implementation date.

When payers start flipping your claims back, you'll wish you would have prepared

Some providers are hedging on whether the transition from 1CD-9 to ICD-10 will actually occur on October 1, 2015. After all, the deadline has already been postponed twice.

The federal governmen t has stated the reason for the delay to 2015 was that small providers were not ready. Unfortunately, the 12 months of extra prep time might not do much to make providers any more prepared for the change.
