ICD-10: What’s in it for the Physicians?

ICD-10: What’s in it for the Physicians?

Recently, I wrote about how important it is to get ready for dual coding. That is critical for any successful transition to ICD-10. This month, let’s focus on physicians, who are another driving force in this area.

One of the issues that keeps cropping up when working with hospitals and physician groups is that physicians are asking “what’s in it for me?” That is quite an obstacle to overcome. Granted, there is no monetary return associated with ICD-10 for the practitioners, but they need to understand that the diagnoses and the conditions they are treating need to be supported by medical necessity – and according to the Centers for Medicare & Medicaid Services (CMS) medical necessity is the “overarching criterion.” Without evidence of medical necessity, it does not matter what CPT/HCPCS code is submitted; the claim will not be paid.

When it comes to healthcare I wear many hats that have broadened my perspective on the transformation we are currently experiencing in this industry.

On most days I lead a physician training software company, on others I care for patients as a physician, and in yet others I mentor about a hundred high school children each year who dream of becoming doctors one day. This past month I put on a hat I haven’t worn in some time, having accompanied two family members on two separate ER visits varying from a simple fracture to an unfortunate death.  I was impressed by the care delivered in these two very different ER settings and care scenarios.  I always try to reflect on my experiences across these varying roles, frequently centering myself on the perspective of a very important stakeholder in healthcare, physicians.

HATA Launching ICD-10 Readiness Survey

HATA Launching ICD-10 Readiness Survey

Off the heels of the release of results from the most recent Workgroup for Electronic Data Interchange (WEDI) ICD-10 readiness survey, the Healthcare Administrative Technology Association (HATA) has launched a similar survey of its own, association Executive Director Tim McMullen announced this week.

The HATA survey will query practice management systems vendors regarding their readiness.

“Practice management systems are a vital component in the transition to ICD-10,” McMullen said. “This is the first time in which data just on practice management system vendors (and) their ICD-10 readiness has been gathered.”

October 2015 ICD-10 Compliance Date: What, Me Worry?

Written by   | Monday, 20 October 2014 16:00

During the first quarter of 2014, health systems worked very diligently to meet the Oct. 1, 2014 ICD-10 deadline. The level of preparedness among providers varied from high to low. In early spring, however, the compliance date was moved to Oct. 1, 2015, and the response from the healthcare industry varied – the question was to move forward as planned, or  to slow down or stop the preparations?

Now that summer is behind us, many providers are still unconcerned about ICD10 compliance – some may be thinking that hey, the compliance date is still far away, next year, and they don’t need to worry about it just yet. But with the fall of 2014 having arrived, the dynamics have changed:

Hospital Computer-Assisted Coding Poised for Strong Growth - #ICD10 #ICDRemediator #ICD10Matters #HealthIT

Hospital Computer-Assisted Coding Poised for Strong Growth

Computer-assisted coding applications have the highest projected growth potential among U.S. hospitals, according to the latest data from HIMSS Analytics.

The organization evaluated 25 support service applications used most by hospitals across the country in terms of their market penetration compared with their projected sales volumes. The HIMSS Analytics’ Essentials of U.S. Hospital IT Market (Autumn 2014 edition) report found that computer-assisted coding applications are installed in fewer than 20 percent of hospitals and have a historical compound annual growth rate of 20 percent or greater.

“In preparation for the shift to ICD-10, it is no surprise that the sales potential for coding solutions is high,” said Lorren Pettit, vice president of market research for HIMSS Analytics. “The findings suggest that market opportunities are ripe for vendors with computer-assisted coding solutions.”

Funny ICD-10 Codes Have Ruined the ICD-10 Branding

The people at online physician community, QuantiaMD, recently sent me a list of the top 3 “Crazy ICD-10 Codes” that they got from their community. It was quite interesting to learn that when they asked their community for these codes, they yielded double the participation the company typically sees. No doubt, physicians have globbed on to these funny and crazy ICD-10 codes. I’ll be honest. I’ve gotten plenty of laughs over some of the funny ICD-10 codes as well. Seriously, you can’t make some of this stuff up. Here’s a look at the top 3 crazy ICD-10 codes they received (and some awesome color commentary from the nominators):
1. W16.221 – Fall into bucket of water, causing drowning and submersion. I didn’t realize mopping the floor was so dangerous!
2. 7. Z63.1 – Problems in relationship with in-laws. Really, Who does not?
3. V9733xD – Sucked into jet engine, subsequent encounter. Oops I did it again.
While these codes are amazing and in many respects ridiculous, they’re so over the top that they’ve branded ICD-10 as a complete joke. For every legitimate story about the value of ICD-10 there have probably been 10 stories talking about the funny and crazy ICD-10 codes. You can imagine which story goes viral. Are you going to share the story that talks about improvement in patient care or the one that makes you laugh? How come the story about their being no ICD-9 code for Ebola hasn’t gone viral (Yes, ICD-10 has a code for Ebola)?

ICD-10 Adoption May Increase Paper Claims and Healthcare Providers' Costs.

Could ICD-10 increase paper claims?

Without a comprehensive risk mitigation strategy, the transition to the ICD-10 codeset has the potential to drastically affect the smooth flow of claims processing and reimbursement. Most of the news about this transition has focused on healthcare IT systems not being ready. But what about the providers? There is always risk in any major transformation. Managing it is the key to a successful implementation.

Former MGMA board co-chair explains how medical groups have changed in recent years and what he expects in the near term

Operating costs, ICD-10, keeping pace with new technologies, and surviving in a world of changing payment models — those are just some of the challenges of running a modern medical practice.
So it’s fitting that they are also among the topics slated for discussion at the Medical Group Management Association annual conference Oct. 25-29 in Las Vegas.
Leading up to the show, William Henderson, former MGMA board co-chair and current chair of the MGMA CEO search committee (the association is currently vetting candidates to succeed former CEO Susan Turney, MD, who departed last month), spoke with Healthcare Finance News about the key issues that are keeping association members busy, how medical groups have changed in recent years and what evolutions he expects in the near term.
